  • Responsible for providing care coordination (treatment planning, assessment, monitoring, educating/linking) to individuals identified in the Transitions to Community Living Initiative Special Populations (TCLI) by targeting those chronic, unresolved, or complex physical, behavioral health, and social determinant needs
