Supervise 8 Care Managers, with support in delivering Tailored Care Management, coordinate with Care Managers about caseload and any escalating cases when complex needs arise, ensure all clinical documentation meets state, Eastpointe and Medicaid requirements, consistently evaluate care management services and ensure implementation of care plans, provide information and assistance in obtaining and maintaining community based resources and social support services, ensure completion of the require contacts.

Work with assigned members on caseload. Monitor hospital admissions and discharges. Participate in discharge planning and follow up between providers to improve engagement in treatment and effective crisis planning to reduce probability of readmission. Make appropriate contacts and complete necessary assessments.

School Counselor at Alternative School. Assist students who are overage, out of home placement, or transitioning from hospital or YDC back into public school to either receive a high school diploma or matriculate to their correct grade. Assist in providing social, emotional and academic interventions for student success.

• Partnered with school administrators, teachers and parents or guardians to provide students with appropriate and adequate counseling support in school.  10th grade Coordinator and maintain caseload of 10-12th graders in assigned alphabet. 

• Conducted therapeutic individual and family therapy sessions.
• Guided clients in effective therapeutic exercises integrated from Cognitive Behavior Therapy and Dialectical Behavior Therapy (DBT).

• Provide counseling, consultation, and advising for the socio-emotional and academic needs of 9th -12th grade students within the assigned alphabet.

• Work weekends completing group therapy sessions and psychosocial assessments for patients with mental illness and/or substance abuse disorders who have been admitted to Holly Hill Hospital.

• Served as the liaison for the Child Family Support Teams in Nash- Rocky Mount Schools.
• Responsible for screening children for mental health, substance abuse or developmental disabilities and provide referral choices to the families.
• Followed up with consumer progress and assisted in the planning of the Person Center Plan.
