  • The Population Health Specialist I is part of an inter-disciplinary clinical care team that helps increase access
  • to health services through ongoing patient outreach and engagement. Primarily, the Population Health
  • Specialist I serves as the point of contact for scheduling Hospital Discharges. Other duties assigned include
  • performing outreach to newly attributed Medicaid beneficiaries, assisting in the maintenance and
  • engagement of active medical provider panels and reviewing patient care gap lists from various reporting
  • portals and software, for performing outreach in alignment with the organization’s current quality and
  • productivity goals.
  • Minimum Qualifications
  • Able to communicate effectively. Interacts in a friendly, professional manner with a wide range of staff, physicians.
  • Receive, track, process, and schedule post-hospital discharge appointments according to WNCCHS’
  • policies and procedures. Processing includes retrieving medical records from hospitalizations using
  • Care Everywhere, Cerner and CareQuality, to ensure their availability in the EHR for clinical review
  • in scheduled visits.
  • Perform timely follow-up to hospital discharge no-shows and cancelations, using the “Pop Health
  • Hospital Discharges – NoShows or Canceled” Report in Analytics Catalog
  • Print and mail Emergency Department/Emergency Room follow-up letters to patients.
  • Perform ongoing outreach to encourage established patients to engage in primary care, for
  • preventative health care and managing chronic diseases.
  • Perform ongoing outreach to newly assigned Medicaid beneficiaries, using information obtained through NCCHCA’s box.com and HealthEC portals.
  • Perform Dismissals from OCHIN Epic in accordance with the organization’s Dismissal protocol.
  • As required, collaborate with other members of the primary care team including Care Managers,

  • Review outreach lists for each Prepaid Health Plan (PHP) and implement a plan for prioritizing those patients experiencing significant SDOH barriers, high numbers of care gaps, or at high risk for hospitalization.
  • Retrieve patient visit agendas from insurance payer portals, and upload to the patient’s chart for clinical teams to reference and address outstanding care gaps (e.g. medication reconciliation, suspect conditions, preventative screenings etc).
  • For newly assigned patients, share information as needed about the health center, available providers, and services and what can be expected at first visit.
