The In Reach Coach is responsible for Diversion and In Reach activities for the TCL Special Healthcare Population with Serious and Persistent Mental Illness (SPMI) residing in Adult Care Homes, State Hospitals, Correctional Institutions and various community settings by providing education about community resources, services, supports and TCL permanent supportive housing. The In Reach Coach ensures members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that helps to establish positive outcomes for members and facilitates community integration.

• Established partnerships with external organizations offering complementary services that enhanced the overall support network available to clients.
• Facilitated group therapy sessions to encourage peer engagement and shared learning experiences among clients.
• Provided empathetic guidance to clients experiencing mental health crises, deescalating situations and connecting them with appropriate support resources.
• Improved client mental health by conducting regular assessments and creating personalized care plans.
