1:1 care with a 3 year old. DX- West syndrome Client was non ambulatory and had seizure activity. Monitored seizures, ADLS, and administered clients medications.

• Assessed and assisted patients with diverse range and comorbid chronic diagnoses, including chronic obstructive pulmonary disease (COPD), asthma, congestive heart failure (CHF), cerebrovascular accidents (CVA), diabetes and sepsis.
• Coordinated and performed patient admissions, transfers and discharges.
• Oversaw medication administration, monitored distribution and documented responses.
• Worked with hospice patients and directly with hospice dealing with end of life care.

• Assessed patients' temperature, pulse, and blood pressure and recorded in electronic medical records.
• Analyzed patient assessments and laboratory data to bring any issues to attention of physicians.
• Documented vitals and medical histories in patient charts when assessing patients and communicated patient information to physicians and other nurses for care continuity.
• Provided compassionate end-of-life care, supporting patients and their families through difficult times.

• Monitored residents, administered medications, Daily wound care performed.
• Worked with Hospice with helping residents transition into end of life. Helped with consoling families and being there for the family.
• Supervise CNAs and assist the CNAs with daily care.
• Collected, checked and filed resident charts and personnel records.
• Managed well-organized system of closed and open medical records with ready availability to interdisciplinary personnel.
• Pulled patient records and transferred information to appropriate parties.
• Safeguarded medical records to maintain patient confidentiality.
• Released information to persons or agencies according to regulations.
• Returned incomplete records and charts to appropriate personnel for correction.
